Personally, I think the most successful way, as far as I've discovered thus far, to make pasties is by getting a stiff piece of felt (like the ones they sell in small sheets approximately 7" x 10" at JoAnns or Hobby Lobby- they've even got cute glittery sheets!) and cutting it into the shape you wish. I'd start with a basic circle until you get more comfortable experimenting. Essentially, this is how you do it:
1) Pick your sheet: color, pattern, etc
2) Measure the approximate diameter of your areola, and make the circle a few centimeters larger, to leave room for cutting in the next step. It's always to go bigger than smaller! There's nothing worse than a too-small pasty revealing your bits.
3) Cut a straight line half-way into your circle, so that it it goes straight to the center of the circle.
4) Overlap the edges where you've cut in order to make it slightly cone-shaped. It's your own preference on how much of a point you want. To have a steeper cone, make the circle larger when first cutting.
5) In order to secure your overlap, make sure you have a hot-glue gun ready. Simply just glue down the overlap, and viola! A slightly coned pasty! Just repeat to make the second.
6) This is the fun part. You can accessorize your pasties in whichever way you wish. It's truly unlimited as to what you can do. You can cover your pasty in glitter glue, which is my own personal preference, and cover it in sequins while its still wet, you can add feathers around the brim, or even beading. If you wish to add tassels, just cut a SMALL h*** in the center, and thread a homemade or premade tassel of some sorts through it, and tie it in the back. It's all up to you to add your own personality to it!

As for making the pasties stick successfully, carpenters double sided tape works wonderfully! You can find this in any home improvement of any major retailer, and for decent price. Target, Ace, Home Depot, etc...

I looked this up and found some "how to's" on the web.  It wasn't hard to find, but I wonder if the craft store sheet foam material suggested will wear down over time?  If you have a nearby naughty lingerie store that sells them, you could try to get a closer look at how they're made and see what the base material is that will be against your skin.  Good luck!  (BTW, I'm too damn flat to wear them and have it look anything but SAD, dammit!)  :)
