I don't think it's just an East Coast thing. I live in Cali and I've heard it called the Monroe. And I want one, too, but I have the same hesitation about it sticking out so far! I just checked out a couple of videos on youtube and the reason they use long posts is to account for the swelling, then when the piercing totally heals, you can put in a shorter post.

The post starts out long to compensate for swelling. After about two weeks you can downgrade to a shorter post, and then so on and so forth. I have so far down graded twice (I have had it about 2 years I think). I play hockey, so I had to have something pretty short and flush with the skin.
Anyway! Back on topic! You change the post so it doesnt catch on your teeth once everything is back to normal.
It heals fast and with a minimum of pain and it is easy to care for. Pain is minimal, but I am a needle addict when it comes to piercings and tattoos so it is all relative.
I have a rhinstone in mine and it is barely noticeable (relatively speaking) and I have had people tell me that even though they dont like piercings, they like the way the stone looks.

they pierce it originally with a longer post because your lip needs room to swell and then heal. of course their are different length posts, you can probably get them where you're getting pierced. the initial piercing, I'm sure, doesn't hurt because the needls are nice and sharp and lubricated. it's the healing that's a b****. I've never had one, however my septum and both nostrils are pierced.
I'm pretty positive a micro dermal wouldn't work on the lip, because the skin is thinner. Keep in mind that a lot of micro dermals are rejected by the body, and if it's not rejected it's basically permanent. they slice the skin an insert an "anchor" that has holes in it to allow the skin to heal on and in and around it... ouchies... no thanks...

My friend who does tattoos and piercings says that dermal is possible. I've been thinking of getting one my self..=)
First off, find a good piercer to do it. They will inform you that the post will be longer due to swelling, you need to go back and get s horter post after healing. I have a dermal as well as Marilyn and they both have thier own plus and minus to them. As far as pain, that is relative to the person. :( But dermals are harder to take care of then the Marilyn.
I say go 4 it I have mine done and have had it 4 years. It does not hurt that bad & the only reason they put a long one in is for the swelling at first, after it is all healed up then yes you can change it to a shorter one :) hope this helps a lil bit.
