Pg. 69, Hey, that's me! I think this would be a great opportunity to introduce myself, my name is Wink Holiday. Check out my profile on PinupLifestyle.com if you'd like to work together sometime, I have access to great photographers that can produce beautiful images of me to grace your pages.

How did this turn out Wink? Did you ever write them?
P.S. Moving this thread to THE SPOTLIGHT! :)
IN an effort to be genuinely helpful/realistic and not to be a killjoy or unsupportive of you, I know they get kajilions of gals contacting them who want to model for them.
I don't find them to be THAT computer oriented. That's why I suggested the letter rather than the email. I'd make a pretty, cute letter and enclose a few small prints of your most flattering shots that you feel are up their alley, and then say I'd love to model again and mention as sweetly as possible that you didn't get credited before. Those guys know the model should be credited. If they realize they slighted you, and you seem professional, sweet and appealing (as a model), you might turn this to your advantage. Then stay in touch periodically without being too pushy.
I would also try to write to the person who edited the article you were in, not the editors of the entire mag who have a billion things on their minds. And be chipper and agreeable. (Which I am sure you were, I'm just stating it for the record.) They are practically harassed by contacts from would-be models, and if you know this going in, you may be better prepared to deal with them.
